you have in your family? We are from Mexico originally and we take our family values highly, everything is about family
NutritionWhat healthy meals do you eat? I cook using fresh vegetable for my family and we mainly eat our traditional foods.
Do you have problems to buying healthy foods? Sometimes because we try to eat healthy and buying healthy foods can be expensive.
Do you have any food restrictions? Yes, I have to watch my salt intake and my husband his cholesterol intake

Describe how comfortable the bed you sleep on?. Our bed is very comfortable we just bought a

EliminationHow often do you have a bowel movement? Every 2-3 days
Do you have any disease of the digestive system, urinary system or skin? No, I don’t have any problem with elimination. Do you urinate

frequently at night or during the day? No, even though I take a fluid pill
Activity/ExerciseHow often do exercise in the week? I don’t exercise on a regular basis due to a busy schedule.
What type of exercise do you do?  I don’t have any regular exercise. I walk with my family if am not tired.
Do you have any disease that will prevent you from exercising? No I don’t have any disease to prevent me from exercising.
Cognitive
What level of education do you have? I have a college education.
How well did you do in school? I did  well, I was a B student
Do you have any problem understanding other people?. No I don’t have problems understanding others.
Sensory-Perception
    Would you consider all your senses fully functioning? Yes all my senses are functioning well.
    How do you tell? I just had my annual physical and all my senses are working well.
    Do you have any problems in any of your senses? No I don’t have any problem with my senses

Self-Perception Are you confident in you self? Yes, I am very confident in myself.
What are your goals in life? I plan to return to school to get my master degree as a social worker
Describe this person’s feeling and state of mind? The person I am interviewing appears very happy with her life.
